This form saves reports into the database and makes them available to authorised users under the Barrier Reports module. Each submission receives a tracking code and can be reviewed, escalated, resolved or closed.

The form avoids forcing users to disclose identity or contact details.

Sensitive categories can automatically be marked for escalation.
Dashboard signals
Reports are structured by category, district, facility, source channel and status.
Offline pathway
Later, reports can queue locally using IndexedDB until connectivity returns.
